i've never heard of them canceling the scores of everyone just because of a slight error. the collegeboard does a lot of research and preliminary testing, running questions by experts, and that sort of thing. So there's very rarely anything wrong with a test.
i did hear of one time a student challenged a math question, saying that 2 answer choices were correct. the collegeboard decided he was right, and they changed the scores of all the people who had taken the test. If people had gotten the question wrong before, but now the answer was right, they just amended the scores to reflect that.
